Citizen Service Request Form
The Citizen Service Request is a way for citizens to relay concerns to the City. We encourage you to include your email and/or phone number in case we have questions about your request. We will either respond directly or forward it to the appropriate City department for action.
Note: IF THIS IS AN EMERGENCY REQUIRING THE FIRE, POLICE, OR RESCUE PLEASE DIAL 911 IMMEDIATELY.
